Position: Advanced Healthcare Assistant
Salary: £24,071 to £25,674 per annum, pro rata

About the Role
We are looking for an Advanced Healthcare Assistant to be a part of our Community Nursing Team based in the Maidstone area.
Community Nursing Teams provide interventions for housebound patients aged 18 and over delivering personalised nursing care to those with complex and long-term conditions. Our service aims to enable patients to stay safe and well in their chosen environment.
Advanced Healthcare assistants provide care to people at home and in residential care homes, as part of a care plan agreed with local nursing teams.
As a healthcare assistant, you’ll:
Work as part of a team to provide care to local patients and families monitor the quality of care they’re receiving pand rovide feedback to managers on the quality of care.
Your patients could be any age, but they’ll often be elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities.
As a Band 3 Healthcare Assistant you will work as part of a team to keep people safe and well at home. You will carry out care plans designed to support people living at home, in care homes or in other settings and make sure that people who use our services have a good experience.
Additional Working Pattern: Monday – Friday 8-4. 1 Weekend in 4
Main Duties
Band 3 Senior Health Care Workers, are expected to be kind and responsive but professional and informative and contribute to the quality of the services provided within the community team across the health economy by:
- Keeping the people who use our services as safe as possible through the use of sound clinical skills and effective risk assessments escalating concerns to Registered Healthcare Professional
- Ensuring the best possible clinical outcomes by using up-to-date skills and adhering to evidence-based policies and procedures within the scope of assessed competence
- Ensure the people using our services have a good experience by respecting, empowering and working in partnership with people throughout the care planning process
- To contribute to the ongoing implementation and evaluation of delegated caseload and reporting all clinical and behavioural outcomes to delegate nurse or therapist to support re-assessments
- To undertake, and report back on delegated clinical activities that have previously been signed competent. E.g. baseline vital signs, routine urinalysis, glucose monitoring, wound management, falls prevention awareness, equipment provision, increasing independence through activities of daily living
- To actively work as a member of a multi-professional team to provide high-quality care to patients.
- To plan and implement straightforward tasks and work guided by standard operating procedures and protocols and undertake roles/duties as delegated by a nurse/therapist.
- To undertake clinical interventions utilising a variety of modes, including planning and leading group and individual therapy as part of packages of prescribed care after achieving appropriate competencies delegated by a nurse/therapist.
Qualifications
- Relevant level 3 qualification (NVQ/ QCF Diploma or certificate of care / Advanced apprenticeship) or equivalent or working towards Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C or equivalent in English and Maths)
Experience (Essential)
- Minimum of 2 years experience as a Healthcare Assistant/ Worker in a health or social care setting Experience working as part of a team Experience with the use of Microsoft Office and a variety of IT packages (EPay / E-roster/ AT-P/ CIS)
Desirable
- Experience in community nursing environment
Knowledge (Essential)
- Awareness of developments in the NHS Knowledge of up-to-date clinical practice. Understands the role and remit of Unregistered worker
Skills and abilities (Essential)
- Sound communication and interpersonal skills (verbal and written). Ability to organise own work Ability to cope with change Ability to use own initiative Moderate IT skills.
Personal attributes (Essential)
- Empathetic Supportive Ability to work under pressure Flexible and adaptable Valid driving licence and access to a car for work purposes.
