Position: Enhanced Care Support Worker

Salary: £22,816 to £24,336 per annum, pro rata

Job Type: Full Time, Permanent

About the Role

As an Enhanced Care Support Worker, you will support, supervise and care for patients who may be at risk of harm from falls, deterioration or isolation. Enhanced care can either be provided on a one to one basis or as part of a cohort. This may include people with acute delirium, people with mental health needs, people living with dementia, people with a learning disability, autistic people.

This rewarding position of Enhanced Care Support Worker will develop and evolve with our collaborative team working approach to ensure we meet the advancing demand of patients with additional needs.

You will be required to work shifts including evenings, nights, weekends, and bank holidays. Based within a core team, you will be assigned to a patient/s for the shift and therefore will work across all wards and departments within our Trust, assisting in patient care under the supervision of registered healthcare professionals.

Main Duties

  • In the role of Enhanced Care Support Worker, you will be expected to provide high-quality enhanced care and supervision support that respects people’s dignity and is inclusive of their individual needs.
  • Assist in providing patient care by the individual care plan and the wishes/choice of the patient by instructions and training received, reporting any changes in condition promptly to the Registered Nurse and Enhanced Care and Support Team lead.
  • Provide patient 1:1 care and support through observation, interaction and de-escalation skills aiming to reduce the risk of patient harm and enhance patient experience.
  • Contribute and be actively involved in the assessment, planning, implementation and evaluation of care. Contribute to risk assessment and risk management planning by identifying and reporting issues of concern to the named nurse/nurse in charge
  • Ensure that patient confidentiality is respected at all times, including verbal and written information.
  • Ensure that all care needs, support, and supervision are person-centred and inclusive, familiarising yourself with patient information and care plans is necessary for the delivery of appropriate patient care and support.
  • Maintain a safe clinical environment, prevention, and control of infection by ensuring compliance with policies, standards and procedures and undertaking delegated tasks to contribute to the safe and smooth running of the ward and service. 
  • Work cohesively with all members of the ward team ensuring that the very best care and support to patients is provided at all times. Communicating effectively with patients, relatives/carers and members of the multidisciplinary network of professionals regarding all aspects of care. 
  • Help to maintain a safe environment for patients and staff, reporting any hazards, which might endanger others to the Nurse in charge and/or where applicable take appropriate action to remove these. 
  • Make good use of clinical supervision to discuss casework, make action plans, and reflect on and implement best practices.

Qualifications

  • NVQ Level 3/QCF Level 3 in Health or Social Care or equivalent or working towards.
  • GCSE A-C in both English Language and Mathematics or equivalent in these two subjects e.g., Certificate  Ability and willingness to undertake appropriate training  
  • Basic IT skills
  •  Able to demonstrate literacy and numeracy skills

Desirable

  • Completion of Care Certificate 
  • I.T. training

Knowledge and Experience

  • Substantial evidence of experience as a nursing/care assistant in a health or social care setting, either public, private, or voluntary sector  
  • Understands the need for strict confidentiality. 
  • Knowledge of healthcare work and policies and procedures  

Desirable

  • Experience of working in an Acute Hospital setting  Working knowledge of mental health problems, and eating disorders  
  • Experience working with people with a learning disability and/or autistic people.  Experience working with people living with dementia
